Certified address request

Salt Lake City requires a Certificate of Address (also known as a Certified Address) for many types of property work. You’ll need one for building permits, subdivisions, consolidations, and other changes that affect a property’s address. You also need one whenever a new mailing address is created or updated, including when suites or apartment numbers are added or removed. Utility companies may ask for a certified address before they can start service.

A Certificate of Address costs $13. Your application must include:

  • The property owner’s name
  • The address
  • Contact information for the person requesting and paying for the certificate

What you’ll need

To determine an address, you must provide one or more of the following items:

  • The Salt Lake County parcel number for the lot.
  • The subdivision or plat name and lot number(s), if applicable
  • Any known or approximate address(es) for the property or project area
  • A list of all buildings and all residential units and/or commercial suites on the property, both existing and new
  • A site plan that includes:
    • Parcel boundaries with dimensions
    • Building footprints for all structures (new, proposed, and existing)
    • Front doors and main entry points
    • FDC points, if applicable
    • All streets the property fronts on
    • A drawing that is accurate and to scale (with a scale bar or ratio) or a measurement from a neighboring lot boundary to the center of the front entry
    • If you don’t have a site plan, or you are not applying as part of a building permit, please contact us

Important: We cannot issue a Certificate of Address for nonconforming units or structures unless they are being legalized through a building permit. You may be required to supply additional documentation. We do not guarantee a desired address will be approved, or an existing address will be maintained.

How to apply

You must apply for a Certificate of Address online or by email. We do not offer hard-copy forms, accept walk-ins, or process payments in person or over the phone.
