Before you start
- Fraud – Attempting to obtain something of value through willful misrepresentation.
- Waste – Squandering money or resources, even if not explicitly illegal.
- Abuse – Behaving improperly or unreasonable or misusing one’s position or authority.
Steps
1. Report Fraud, Waste, or Abuse
- Visit the independent website.
- Click the “Submit” button to report an incident.
- Detail the incident of fraud, waste, or abuse and upload any supporting documentation.
2. Create a PIN for your report
- Create a PIN at the end of the incident report.
- Keep this PIN and the report/case number that will be created on the confirmation page.
- Please write them down and do not lose these numbers as they cannot be recreated!
3.Use PIN and Case Number to track report
- Use the PIN you created and the report/case number to log in to the independent website.
- Follow-up or get the status of the existing report.
What’s next
- The City’s Fraud, Waste, and Abuse Committee will review all submitted incident reports and determine course of action.
- If an investigation is warranted, Administrative Internal Audit and/or Human Resources will be assigned to complete the investigation and report back to the committee.
- If any follow-up information is needed for an investigation, an internal auditor may contact you.
- Use the report/case number and PIN to follow-up or get the status of the existing report.
