Grievance Procedure
The following procedures apply to all grievances filed under Title VI of the Civil Rights Act of 1964 and related statutes regarding any program or activity implemented by SLC. Any person who believes they, or any specific class of persons, have been subjected to discrimination on the basis of race, color, religion, national origin (including limited English proficiency, sex, or age, may by themselves or by a representative file a written complaint as described herein.

Filing a Grievance:
- Grievances must be submitted to the SLC Title VI Coordinator as soon as possible but no later than 180 calendar days after the alleged discriminatory action. Click here to download and print a copy of the grievance form. To submit a grievance directly through our online portal click here.
*Alternative means of filing a grievance, such as personal interviews or a tape recording of the complaint, may be made available for people with disabilities; complainants should request assistance from the SLC Title VI Coordinator (contact information provided below).
- Upon receipt of the grievance, the SLC Title VI Coordinator will make a determination whether SLC has jurisdiction to investigate the issues presented. If SLC does have jurisdiction, SLC shall confirm receipt and inform the complainant of the investigation process in writing within five (5) business days after receipt of the complaint. If SLC does not have jurisdiction, the complaint will be forwarded to the appropriate agency for investigation and SLC shall issue a non-jurisdiction letter informing complainant where the grievance was forwarded.
- If SLC has jurisdiction to investigate, SLC will conduct the investigation within ninety (90) days of receipt of the complaint. The SLC Title VI Coordinator may reach out to complainant to request additional information or documentation. SLC will respond to the complainant at the conclusion of the investigation with the results.
These procedures do not deny the right of a complainant to directly file a grievance with state or federal agencies. Complaints may be filed in writing directly with the Federal Highway Administration, Utah Department of Transportation, or the U.S. Environmental Protection Agency at the addresses listed below:
