Surface Fault Rupture Study Waiver Form
This waiver is required for properties located within or near a Surface Fault Rupture Study Area when the property owner chooses not to obtain recommended geotechnical or geological studies.
Overview
Salt Lake City notifies property owners when their property is located in or near a Surface Fault Rupture Study Area. The City recommends obtaining professional reports, such as a soil engineering report or engineering geology report, before proceeding with construction.
Owner Decision
By signing this waiver, the property owner acknowledges that they have chosen not to obtain the recommended Special Study Reports and will not implement any related recommendations into the construction project.
Liability Waiver
The property owner agrees to hold harmless Salt Lake City Corporation from any and all liability related to damages or losses resulting from proceeding without the recommended studies.
This includes any risks associated with construction in a Surface Fault Rupture Study Area.
Important Note
Choosing not to obtain geotechnical or geological studies may increase the risk of structural damage or failure due to ground movement or fault activity.
