Overview
Applicants can submit this questionnaire along with a permit application. The information is used to evaluate roof loading, existing conditions, and whether additional engineering is required.

Roof Information
- Type of existing roofing materials
- Type of proposed roofing materials
- Type of existing roof deck
Key Review Questions
- Will existing roofing materials be removed?
- If no, provide the number of existing roof layers
- Will the additional roofing weight exceed 3 pounds per square foot (psf)?
- Does the building have unreinforced masonry (URM) parapets or rooftop appendages (chimneys, tanks, signs, etc.)?
When Engineering Is Required
Structural Load Evaluation
If the added roofing weight exceeds 3 psf, an evaluation report from a licensed Utah engineer is required to verify that the existing framing can support the additional load.
Unreinforced Masonry (URM)
If the building includes URM parapets or rooftop appendages, engineering design and calculations are required for bracing and wall anchorage to tie the roof to supporting walls.
Exception: One- and two-family dwellings may be exempt.

Unreinforced Masonry (URM): Masonry construction (brick, stone, or similar materials) with little or no steel reinforcement.

Additional structural or engineering documentation may be required based on project conditions. Failure to provide required information may delay permit approval.
