Contractor Transfer Request Form
The Contractor Transfer Request form is used to notify Salt Lake City of a change in contractor associated with an active building permit.
Overview
Property owners or authorized agents must submit this form when replacing an existing contractor on a permitted project. The request updates the permit record to reflect the new contractor responsible for completing the work.
Information Required
- Project address, permit number, and project name
- Previous contractor’s business name, license number, and contact information
- New contractor’s business name, license number, and contact information
- Last day of work performed by the previous contractor
Owner Responsibility
The property owner (or authorized agent) must formally notify the Building Official of the contractor change and sign the request confirming the transition.
New Contractor Responsibility
The new contractor must agree to comply with all terms and conditions of the existing permit and perform all work in accordance with applicable laws.
Submittal Instructions
- Include a copy of the new contractor’s license

Important Note
Transfer of a building permit does not include transfer of any copyright-protected construction documents.
