Absence Management
The Absence Management Team provides assistance and administers leaves of absence for the entire organization of Salt Lake City Corporation. This includes Family Medical Leaves (FMLA), Department Leaves of Absence (DLOA), Parental Leave, and Military Leaves. The team also processes Short Term Disability and Long Term Disability claims (LTD).

Services Provided by the Leave Management Team:
FMLA Continuous/Intermittent Leave Administration
Collect and review appropriate forms and documents. Approve or deny FMLA leave requests. Track FMLA leave. Send notification of FMLA status.
USERRA Military Leave
Answer questions regarding military leave. Send notification to SLC employees and their departments. Track military leave. Coordinate employee benefits with Benefits Analyst.
Short-Term Disability
Provide short-term disability claims to employees. Coordinate leave dates, wages, employee information to third party disability vendor, The Hartford.
Long-Term Disability
Send LTD applications to SLC employees. Send LTD applications and supporting documentation to The Hartford for approval. Send notification of LTD status.
AD&D, Illness, or Hospital Indemnity
Provide AD&D, Illness, or Hospital claims to employees. Send claims and supported documents to the appropriate vendors for processing.
Supervisor LOA Training 101
If your management team needs training for LOA, will be more than happy to schedule time to get out to your location and provide training. Training sessions can be from 1 hour to 1.5 hours per session.
